[T]op U.S. ground commander in Iraq telling his officers that some U.S. troops have been their "own worst enemy" because of how they've dealt with Iraqis.

There are two things that strike me about this quote. First, that it even gets media attention. Isn't this concept a given? Maybe not, which goes some distance in explaining the mess in Iraq. The other thought I had was, isn't it a little late in the game? We're what ... three years into the insurgency?
